Filling out the form

Filling out the voter registration form is easier than it may look at first.

It just asks for things like your name and address, where you were born, your age, and if are you a U.S. citizen. It also asks you why you are registering: new voter? address change? change of political party?

Click on the picture of the form below to see the complete form.

You can have a friend or family member help you fill out the form. They will need to fill out section #12.

When you fill out the form, you can choose a political party.

Important!

  • Be sure to sign the form.

  • Put your completed form in the mail so it arrives by the registration deadline.

  • If someone else mails the form in for you, they should give you the stub at the bottom of the form to keep as proof of your vote.
